ABSTRACT

The Cautín River is closely related with the economic development of Temuco city, (38°S; Chile). Existing knowledge of the Cautín River is limited to information about its biological characteristics as a reference for the evaluation and assessment of water quality. The object of this study was to develop taxonomic characterisation of the benthic macroinvertebrates along the main course of the Cautín River, and to study the community structure using correlation analysis between community parameters. To carry out this research, the macroinvertebrate community was studied in 10 sampling sites distributed along the main course of the river. The samples were taken in summer (1997 and 2000), when optimal hydrological conditions existed. Analysis of the samples showed that the benthic fauna was composed of 56 taxa, the dominant group being insects with 48 taxa. Three main sectors were recognised in the course of the Cautín River: high, middle and low. Each sector has restricted-distribution species, while other species are widely distributed along the river. These distribution patterns seem to be influenced by dissolved oxygen concentration, temperature, altitudinal distribution and anthropo-cultural activity, present at every sampling site. Finally, this research provides a first approach to the biology of the Cautín River. Further studies could be planned on the basis of this knowledge to investigate water quality indicators based on macroinvertebrate communities.

Quanto ao conhecimento total do rio Cautín, existem informações limitadas sobre as características biológicas que podem servir de referência para a avaliação da qualidade da água. Este estudo tem o objetivo de caracterizar os macroinvertebrados bentônicos taxonomicamente ao longo do curso principal do rio Cautín e estudar a estrutura da comunidade usando análise de correlação entre os parâmetros dela. Para realizar esta pesquisa, a comunidade de macroinvertebrados foi estudada em dez locais de estudo distribuídos ao longo do rio principal. Esses padrões de distribuição parecem influenciar a concentração de oxigênio dissolvido, a temperatura, a distribuição altitudinal e a atividade antropocultural desenvolvida em todos os locais de amostragem. Finalmente, esta pesquisa fornece uma primeira abordagem biológica do rio Cautín e, de acordo com esses conhecimentos, um estudo posterior pode ser planejado em relação aos indicadores de qualidade da água e com base nas comunidades de macroinvertebrados.

INTRODUCCIÓN: La Organización Mundial de La Salud ha reportado recientemente que el nuevo foco de la pandemia global de la enfermedad Covid-19 es el continente americano. OBJETIVO: Realizar una revisión de la literatura sobre la experiencia internacional de la pandemia Covid 19 y embarazo. MÉTODO: Se realiza una búsqueda de la base de datos PubMed para las palabras clave Pregnancy / Pregnant / Novel Coronavirus / SARS-CoV-2 / Covid-19, desde el 1 de noviembre 2019 hasta el 21 de mayo 2020. RESULTADOS: Un total de 365 artículos fueron inicialmente seleccionados de acuerdo con la estrategia de búsqueda diseñada. El total de artículos revisados de acuerdo con los criterios fueron 42. Las series clínicas seleccionadas acumularon un total de 1098 embarazadas y enfermedad de Covid-19. Las co-morbilidades mas frecuentes fueron hipertensión arterial, diabetes mellitus, obesidad y asma. La mortalidad en relación con el total de pacientes fue de un 1,2 % y la transmisión al recién nacido de 1,7% (15 de 875). CONCLUSIÓN: La información obtenida permite inferir que la presentación clínica de la enfermedad es a lo menos equivalente a la de mujeres de la misma edad no embarazadas. Dada la severidad de la enfermedad por SARS-CoV-2 reportada, las lecciones aprendidas deben ser rápidamente asimiladas y utilizadas en el contexto de la situación nacional epidémica.


INTRODUCTION: The World Health Organization has recently reported that the new focus of the global pandemic of Covid-19 disease is the American continent. OBJECTIVE: To conduct a literature review on the international experience of the Covid 19 pandemic and pregnancy. METHOD: A PubMed database search is performed for the keywords Pregnancy / Pregnant / Novel Coronavirus / SARS-CoV-2 / Covid-19, from November 1, 2019 to May 21, 2020. RESULTS: A total of 365 articles were initially selected according to the designed search strategy. The total of articles reviewed according to the criteria was 42. The selected clinical series accumulated a total of 1098 pregnant women and Covid-19 disease. The most frequent comorbidities were hypertension, diabetes mellitus, obesity, and asthma. Mortality in relation to the total number of patients was 1.2% and transmission to the newborn was 1.7% (15 of 875). CONCLUSION: The information obtained allows us to infer that the clinical presentation of the disease is at least equivalent to that of non-pregnant women of the same age. Given the severity of the reported SARS-CoV-2 disease, the lessons learned must be quickly assimilated and used in the context of the national epidemic situation.

ABSTRACT

El objetivo de este estudio fue evaluar si las medidas antropométricas y demográficas de pacientes con esqueleto inmaduro pueden ser utilizadas para predecir el diámetro del autoinjerto semitendinoso-gracilis (ST-G) en la cirugía de reconstrucción del ligamento cruzado anterior (LCA). Métodos: Serie retrospectiva de 169 pacientes con esqueleto inmaduro (112 hombres, 57 mujeres, edad media 15,8 años) que se sometieron a cirugía de reconstrucción del LCA con autoinjerto ST-G. Se registraron las medidas antropométricas (peso, talla e índice de masa corporal [IMC]), datos demográficos (edad y sexo) y diámetro del autoinjerto ST-G utilizados. Se utilizaron coeficientes de correlación, regresión lineal y t de Student para determinar la relación entre variables antropométricas-demográficas y el diámetro del injerto. Los valores de p < 0,05 fueron considerados estadísticamente significativos. Resultados: El análisis estadístico mostró una correlación positiva significativa tanto para la talla y el peso con el diámetro del injerto (r = 0,483, p < 0,0001; r = 0,248, p = 0,001, respectivamente). Sin embargo, la edad y el IMC no se correlacionaron con el diámetro del injerto (r = 0,098 y p = 0,203, r = 0,009 y p = 0,905, respectivamente). La media del diámetro del injerto en los hombres fue de 8,2mm (rango 6-10) y 7,5mm en las mujeres (rango 6-9), diferencia que fue estadísticamente significativa (p < 0,001). Conclusiones: La predicción del diámetro del injerto ST-G según la talla del paciente es un método fácil y fiable en pacientes con esqueleto inmaduro. The aim of this study was to evaluate whether the anthropometric and demographic measures of patients under 18 years old can be used to predict the diameter of semitendinosus-gracilis (ST-G) autografts in anterior cruciate ligament (ACL) reconstruction surgery. Methods: A study was conducted on retrospective series of 169 patients under 18 years (112 men, 57 women, average age 15.8 years) who underwent to ACL reconstruction surgery with ST-G autograft. The anthropometric measurements were recorded (weight, height and body mass index), demographics (age and gender) and the diameter of the ST-G used. Correlation coefficients, linear regression and unpaired t-test were used to determine the relationship between anthropometric/demographic variables and the diameter of the graft. P values <.05 were considered statistically significant. Results:Correlation analysis showed a significant positive relationship between height and graft diameter (r=0.483, P<.0001), as well as between weight and graft diameter (r=0.248, P=.001). However, age and body mass index (BMI) did not correlate with graft thickness (r=0.098 and P=.203, r=0.009 and P=.905, respectively). The mean graft diameter in men was 8.2 (range 6-10) and 7.5 in women (range 6-9), a difference that was statistically significant (P<.001). Conclusions:Prediction of the ST-G graft diameter according the height of the patient is an easy and reliable method in children and adolescents. Determinar qué porcentaje de una población sin dolor anterior de rodilla tiene un test de Zohlen positivo, además determinar el ángulo Q de esta población y buscar si existe alguna relación entre la positividad del test de Zohlen y alteraciones en el ángulo Q. Material y método: Estudio descriptivo-prospectivo observacional. Aplicación del test de Zohlen y medición del ángulo Q. La población se dividió en 2 grupos: test de Zohlen positivo y test de Zohlen negativo. Cuantificación y comparación de medias del ángulo Q en los dos grupos. Resultados: 90 sujetos evaluados, promedio de edad 20,18 años (18-40). Veinte sujetos (22,2 por ciento) con test de Zohlen positivo. Ángulo Q promedio en los sujetos con test de Zohlen negativo: 14,95°; ángulo Q promedio en los sujetos con test de Zohlen positivo: 16,9° (p < 0,05). Ángulo Q promedio en hombres con test de Zohlen negativo 13,4°; ángulo Q promedio en hombres con test de Zohlen positivo: 16° (p < 0,05). Ángulo Q promedio en mujeres con test de Zohlen negativo: 16,5°; ángulo Q promedio en mujeres con test de Zohlen positivo: 18°, sin diferencias estadísticamente significativas entre ambos grupos. Conclusiones: El test de Zohlen tiene una correlación positiva con el ángulo Q en sujetos de sexo masculino. Dada la correlación entre un ángulo Q alterado y la presencia de dolor anterior de rodilla, en los pacientes que presentan un test de Zohlen positivo sin haber consultado por dolor anterior de rodilla, la prevención primaria de dolor anterior de rodilla puede ser de utilidad...


To determine the percentage of a population without anterior knee pain with a positive Zohlen test, and also to determine the Q angle of this population and to determine if there is any relationship between the Zohlen test and Q angle anomalies. Methods:A prospective observational study was conducted in which Zohlen¿s test was applied and the Q angle was measured. The population was divided into 2 groups: Zohlen¿s positive and Zohlen¿s negative. Q angle was compared in the 2 groups. Results: The study included 90 subjects, with a mean age 20.18 years (18-40), of whom 20 subjects (22.2 percent) had positive Zohlen¿s test. The mean Q angle in subjects with negative Zohlen¿s test was 14.95°, and the mean Q angle in subjects with positive Zohlen¿s test was 16,9° (p<.05). The mean Q angle in men with negative Zohlen¿s test was 13.4°, and the mean Q angle in men with positive Zohlen¿s test was 16° (p < .05). The mean Q angle in women with negative Zohlen¿s test was 16.5°, with a mean Q angle of 18° in women with positive Zohlen¿s test, with no statistically significant differences found between groups. ABSTRACT

Los embarazos múltiples se consideran una entidad de alto riesgo obstétrico. Su incidencia ha ido en aumento debido a la utilización de técnicas de reproducción asistida y el aumento de la edad materna. Se presentan 2 casos de embarazos triples monocoriales triamnióticos, de sexo femenino y masculino. En ambos embarazos se manifestaron complicaciones, principalmente fetales y neonatales, atribuidas a embarazos múltiples descritas en la literatura.


Multiple pregnancies are considered a high-risk obstetric entity. Their incidence has been increasing due to the use of assisted reproductive techniques and increased maternal age. Here, we describe two cases of triamniotic monochorionic triplet pregnancies, female and male respectively. Both pregnancies demonstrated complications, mainly fetal and neonatal, attributed to multiple pregnancies reported in the literature.

ABSTRACT

Objective: To evaluate longitudinal changes in the cardiac function parameters in relation to the hemodynamic worsening in growth restricted (IUGR) fetuses. Methods: The cardiac function was serially studied in a cohort of 46 IUGR fetuses below 32 weeks of gestation by 2-D and Doppler ultrasound parameters. IUGR was defined as an estimated fetal weight below the 10th percentile with a Doppler pulsatility index (PI) in the umbilical artery (UA) above 2 standard deviations of the mean. Longitudinal changes, perinatal deaths and survivors were evaluated. Results: All cardiac parameters were significantly different in IUGR fetuses. Aortic isthmus blood flow index (IFI) and Modified myocardial performance index (Mod-MPI) showed the clearest longitudinal changes and temporal deterioration. Conclusions: IFI and Mod-MPI showed consistent longitudinal changes in IUGR fetuses. These 2 markers could be added to the DV to evaluate the fetal deterioration and indicate the optimal time of delivery.


Objetivo: Evaluar cambios longitudinales de los parámetros de función cardíaca en relación al agravamiento hemodinámico en fetos con restricción del crecimiento intrauterino (RCIU). Métodos: La función cardíaca fue estudiada en una cohorte de 46 fetos RCIU con menos de 32 semanas de gestación con parámetros 2-D y ultrasonografía Doppler. Se definió RCIU como un peso fetal estimado bajo el percentil 10 asociado a un índice de pulsatilidad (IP) al Doppler de la arteria umbilical (AU) por debajo de dos desviaciones estándar del promedio. Se evaluaron cambios longitudinales, mortalidad y sobrevida perinatal. Resultados: Todos los parámetros tuvieron diferencias estadísticamente significativas en fetos RCIU. El índice de velocidad de flujo del istmo aórtico (IFI) y el índice de función miocárdico modificado (IFM-mod) mostraron los más evidentes cambios longitudinales y deterioro en el tiempo. Conclusiones: El IFI y el IFM-mod mostraron consistentes cambios longitudinales en fetos RCIU. Estos dos marcadores podrían ser usados junto a la medición del ductus venoso para evaluar el deterioro fetal y la indicación óptima del momento del parto.

ABSTRACT

The objective of the present study was to identify metabolic, cardiovascular and autonomic changes induced by fructose overload administered in the drinking water of rats for 8 weeks. Female Wistar rats (200-220 g) were divided into 2 groups: control (N = 8) and fructose-fed rats (N = 5; 100 mg/L fructose in drinking water for 8 weeks). The autonomic control of heart rate was evaluated by pharmacological blockade using atropine (3 mg/kg) and propranolol (4 mg/kg). The animals were submitted to an intravenous insulin tolerance test (ITT) and to blood glucose measurement. The fructose overload induced a significant increase in body weight (~10 percent) and in fasting glycemia (~28 percent). The rate constant of glucose disappearance (KITT) during ITT was lower in fructose-fed rats (3.25 ± 0.7 percent/min) compared with controls (4.95 ± 0.3 percent/min, P < 0.05) indicating insulin resistance. The fructose-fed group presented increased arterial pressure compared to controls (122 ± 3 vs 108 ± 1 mmHg, P < 0.05) and a reduction in vagal tonus (31 ± 9 vs 55 ± 5 bpm in controls, P < 0.05). No changes in sympathetic tonus were observed. A positive correlation, tested by the Pearson correlation, was demonstrable between cardiac vagal tonus and KITT (r = 0.8, P = 0.02). These data provided new information regarding the role of parasympathetic dysfunction associated with insulin resistance in the development of early metabolic and cardiovascular alterations induced by a high fructose diet.

ABSTRACT

Objetivo: Establecer la validez diagnóstica de la identificación del sexo fetal en plasma de mujeres embarazadas durante la primera mitad del embarazo mediante medición de ADN libre fetal (ffDNA) en sangre materna. Método: Se extrajo 20 mi de sangre periférica a 25 pacientes entre las 5 y 15 semanas de gestación, previo consentimiento informado. Se separó el plasma mediante centrifugación obteniéndose ADN genómico total (materno y fetal), y se analizó por triplicado mediante PCR en tiempo real (q-PCR), con partidores específicos para secuencia del cromosoma Y (DYS14). El sexo fetal se confirmó mediante ecografía realizada entre las 20 y 25 semanas. Resultados: 12 pacientes presentaron amplificación para DYS14, siendo diagnosticados como fetos masculinos. La menor edad gestacional de diagnóstico fue 6+4 semanas. En 9 pacientes no se generó señal, estableciéndose el sexo fetal como femenino. Hubo 4 casos en donde no se cumplieron los criterios para definir el sexo fetal, estableciéndose estos como indeterminados (todos ellos antes de las 8+3 semanas). La probabilidad de diagnosticar el sexo a partir del test fue de 84 por ciento (21 de 25 pacientes). Considerando solo los 21 casos donde se pudo realizar un diagnóstico, la ecografía confirmó el sexo en todos ellos, obteniéndose una correlación del 100 por ciento. Conclusión: Es posible diagnosticar el sexo fetal en sangre materna utilizando ffDNA durante la primera mitad del embarazo.


Objective: To validate the use of cell free fetal DNA (ffDNA) in maternal plasma to predict fetal sex in the first half of pregnancy. Method: A prospective observational cohort study of 25 pregnancies between 5 and 15 weeks of gestation was studied (median gestational age of 9+1 weeks). Maternal blood was taken and q-PCR was carried out to detect the multi-copy Y chromosome associated DSY14 gene. The end point was gender as assessed by ultrasound at 20-25 weeks. Results: A Y signal was obtained in 12 patients, so a male fetus was predicted. The earliest signal was at 6+4 weeks. In 9 patients we didn't have any signal, so a female fetus was diagnosed. There were 4 cases where the criterion to define fetal sex was not fulfilled and were classified as equivocal (all of them before 8+3 weeks). The probability to predict fetal sex from the test was 84 percent (21 of 25 patients). However, when the diagnosis of fetal sex is made, there is a 100 percent correlation between the ultrasound and Q-PCR. Conclusion: Free fetal DNA in maternal plasma allows prediction of fetal sex in the first half of pregnancy.

ABSTRACT

Introducción: La Diabetes Mellitus tipo 2 (DM") y sus complicaciones tardías son prevenibles a un costo razonable cuando se implementan programas educativos. Objetivo: Evaluar el efecto de la intervención educativa en el control metabólico de los pacientes diabéticos tipo 2 del Hospital Nacional Arzobispo Loayza en Lima-Perú. Material y Métodos: Diseño del estudio: Experimental. Población: Pacientes con DM2 del HNAL. Los criterios de ingreso al estudio fueron: a) Paciente con DM 2 definida menor, b) edad menor a 8 años, c) seguimiento por consulta ambulatoria > 9 meses. d) Ser residentes de Lima-Perú. Grupo Intervención: 107 pacientes que recibieron la intervención educativa en el Programa de Educación Básica en Diabetes Mellitus (PEBADIM). Grupo Control: 121 pacientes que no recibieron la intervención. La intervención educativa comprendía conocimientos sobre la DM 2, control metabólico, prevención, diagnóstico y tratamiento de manifestaciones tardías, nutrición, ejercicios y uso correcto de medicamentos; a cargo de un equipo multidisciplinario. Mediciones: Los datos fueron recolectados al inicio, 3,6, y 9 meses, las variables de control metabólico comprendieron a la glucosa en ayunas, glucosa post prandial, Hemoglobina glicosilada, e IMC. Además evaluamos las variaciones en la presión arterial, perfil de lípidos, microalbuminuria, proteinuria de 24 horas y depuración de creatinina. Resultados: Las características clínicas y bioquímicas de ambos grupos al inicio del estudio fueron semejantes. El grupo PEBADIM cursó con concentraciones de glicemia en ayunas y glicemia post prandial menores al grupo control desde la primera evaluación hasta el final del estudio (prueba t de student, p<0.05). La proporción de pacientes con glicemia controlada (menor a 110mg por ciento) al inicio del estudio fue similar para ambos grupos (5.7 por ciento vs 3.5 por ciento), incrementándose hasta 485 por ciento en el grupo PEBADIM a los 6 meses versus 33.8 por ciento...

ABSTRACT

La capacidad contráctil del útero puede ser evaluada in vitro por medio del método isométrico. El objetivo de este estudio fue comparar la efectividad de la administración simultánea de oxitocina e insulina en la contractilidad uterina. La comparación entre ambas hormonas se realizó en úteros aislados de 2 modelos experimentales: ratas hembras adultas en diferentes etapas del ciclo estral y ratas preñadas en fase final de preñez. Los úteros de estos animales fueron sometidos, in vitro, a la acción de dosis acumulativas de oxitocina (10-10 M a 10-8 M), insulina (200 mU/ml), y oxitocina más insulina. Los resultados obtenidos indican que in vitro la insulina no modificó significativamente la respuesta oxitócica. Sin embargo, la insulina aumentó la entrada de potasio al miometrio, medida como actividad de la Na-K ATPasa sensible a ouabaína. Este hallazgo indica efectividad de la insulina en el tejido uterino. Además es la primera demostración que insulina actúa en el miometrio en forma similar a lo descrito para el músculo esquelético. El efecto de insulina sobre la bomba de sodio indicaría que insulina no actuaría sinérgicamente con oxitocina

ABSTRACT

Relatamos o caso de uma ursa polar (Ursus maritimus) de 24 anos de idade, que contraiu a infecção chagásica no Zoológico de Guadalajara, em Jalisco, no México, e morreu de cardite chagásica aguda 15 dias após o início da sintomatologia. Os achados histopatológicos são descritos, bem como a presença de triatomíneos (Triatoma longipennis Usinger) infectados por Trypanosoma cruzi coletados a 5 metros do local onde o animal vivia, na cidade de Guadalajara.


We report a 24-year-old female polar bear (Ursus maritimus) who contracted Chagas' infection at the Guadalajara Zoo, in Jalisco, México, and died of acute Chagas' carditis 15 days later. The histopathological findings are described, as well as the presence of triatomids (Triatoma longipennis Usinger) infected with Trypanosoma cruzi collected within 5 meters from the place where the animal lived in the city of Guadalajara.

ABSTRACT

Recientemente se ha descrito al virus de la hepatitis C (VHC) como el agente causal más frecuente de la hepatitis post-transfusional No-A, No-B. La vía más importante de transmisión de le hepatitis C es la sangre. El 50 por ciento de los individuos que padecen una hepatitis viral aguda tipo C evolucionarán a una hepatitis crínica, y aproximadamente el 20 por ciento de estos a cirrosis hepática; por otra parte existen evidencias de que el VHC juega un papel en el desarrollo del carcinoma hepatocelular. Presentamos el caso de una paciente que por complicaciones obstétricas fues politransfundida, desarrollando en el puerperio un síndrome ictérico llegándose al diagnóstico de hepatitis C; describimos la metodología diagnóstica, las características clínicas, evolución y se hace énfasis en las medidas de prevención de este padecimiento.
